THE boss of the Royal Bolton Hospital says he will draft in reinforcements to cope with running the Trust in the wake of a mortality rate investigation.

David Wakefield, the expert brought in by health watchdog Monitor six months ago, is due to unveil his plan to board members today.

He said he would be “bringing in additional resources”, which he conceded meant more people.

It comes after the trust’s acting chief executive Dr Jackie Bene “stepped aside” earlier this week when healthcare information expert Dr Foster claimed there were “significant discrepancies” in the hospital’s data regarding the “coding” of cases of septicaemia.

Dr Foster, which measures mortality rates in hospitals, had found Bolton had recorded 800 cases of septicaemia between March, 2011 and April, 2012, when a similar sized trust would expect to have just 200.

They have taken a sample of 200 of the cases and have reviewed coding practices in 50.

As reported exclusively in The Bolton News yesterday, Bolton Clinical Commissioning Group (CCG), who commissioned the investigation, say the interim findings show “cause for concern”.

The investigation will publish its findings next Wednesday.

If cases have been coded incorrectly it could drastically affect Bolton’s mortality figures, as septicaemia deaths are not included in comparable data.

It could also affect finances, as cases of septicaemia will receive higher payments than other infections. Mr Wakefield, interim chairman at the Trust, said it was an independent review and he had not seen the results.

But he said that as far as he understood it, the investigation was about whether Bolton’s coding practices had adhered to national guidelines.

He added: “The issue is whether the clinical coding is consistent and within national guidelines.

“If we find there is an inconsistent coding then there are implications, but we need to wait for the independent review.”

Mr Wakefield said he and Dr Bene, who has been medical director at the Trust since 2008 and a consultant for 14 years, had decided she should “step aside” while the review was completed.

Dr Bene was temporarily leading the Trust, after the early retirement of former chief executive Lesley Doherty at Christmas, and Nicky Ingham, the director of workforce, has been acting deputy chief executive since the start of January.

Mr Wakefield said in normal circumstances, the deputy would lead the Trust but said these were “extraordinary times” and he “felt the need to bring in support”.

Mr Wakefield, who is also chairman at Milton Keynes Hospital NHS Foundation Trust and divides his time between there and Bolton, said he was currently running the trust.

He added he had cancelled his commitments in Milton Keynes this week and was in discussions to “bring in additional resources to bolster the team”.

Mr Wakefield also said he felt capable of running both trusts, adding: “It is difficult, but with the right executive team it is doable for a limited period.”

A spokesperson for Dr Foster said they were unable to comment until the review was completed.

Cllr Andy Morgan, who sits on the Health, Overview and Scrutiny Committee, called for the leadership at the Trust to be stabilised as soon as possible.

He added: “Whoever is appointed to lead the hospital in the interim should obviously get our full support but this staggering from one chief executive and crisis to the next and the uncertainty that this brings cannot be allowed to continue.

“The chairman needs to start the recruitment process again as a matter of urgency to appoint a new chief executive who can take charge and lead the board and staff out of the current turmoil it is in.

“Only then can the public and more importantly the staff have the stability needed and confidence that brings with regards the future of the hospital.”

Leader of Bolton Council, Cllr Cliff Morris, who was chairman of the Trust for eight years until he “left early” in August, said he did not feel able to comment.

He added: “I am pleased that there is an investigation into the death rates at the hospital, but would like to await the outcome of the report on March 6 before making a further comment.”

“We are going to wait until the report is released next week.”

Council bosses also promised to provide “support” should it be needed.

Bolton Council’s chief executive Sean Harriss, said: “We are aware of the situation and will await the outcome of the report next week before commenting on any detail.

“In the meantime, as was agreed at yesterday’s Shadow Health and Well Being Board, we are intending to work with our health partners in Bolton to provide any support that may be needed.”

At least now the issue has hit the national press. Wouldn't a dispatches investigation from channel 4 be interesting? They could look into the fat controller's role in all of this too. Not panorama though, too biased to labour.

It would seem that nobody is prepared to make any comment at this point! I suspect these fat cats who are complicit in this appalling situation are just praying that none of the fallout blows their way. It's incredible that the police or a national newspaper or both have not shown any interest thus far in a hospital that has corrupt practices further compounded by a culture of cronyism. The RBH is completely target driven, giving frontline staff impossible deadlines with increasingly less resources to achieve the same results. Additionally It claims their will be no impact on patient care. Well it does and it will worsen if these incompetent good for a giggle fat cats are allowed to continue on course.

In light of the growing body of concerns around the governance at the most senior level at the Royal Bolton Hospital and the links and oversight that the local authority have to the Trust is it not time that either Leader of Bolton Council, Cllr Cliff Morris or the incumbent majority party consider how appropriate his position is.

To stay with his role at the local authority in my humble opinion the gentleman must;
• State publically what he knew and when he gained this information regarding the financial crisis dogging the Trust.
• If he is to remain; to make a public statement clearly removing himself from this areas of influence within his remit at the Authority – additionally making clear what these responsibilities are and who will now be picking them up.
Without some form of clear statement I feel that the honourable gentleman’s position may be becoming untenable.

Hardly fair to say that RBH is "completely target driven, giving frontline staff impossible deadlines with increasingly less resources to achieve the same results. ALL NHS Trusts are the same, those at the top not content with making frontline staffs lives a misery by expecting the impossible, but following it up with rude e-mails when the targets aren't met. Bullying and threatening staff, cutting staff numbers, jumping on staff when they can't provide care due to lack of resources, expecting patients to be put in a bed, any bed, to prevent breaches regardless of patient safety, shutting beds to save money then expecting staff to open extra beds despite not having enough staff to provide care etc etc etc is a daily occurence. Retirement is the only escape, unless of course the stress kills you first!!!

I agree all hospitals are the same and it is now a common culture within the NHS to lean on staff. I was just stating that the RBH has other issues that are compounding this matter and therfore leading to further misery.
Nonetheless you are spot on with what you have to say.

I hope that Dr Bene has stood aside only from CEO duties and not from her clinical work .She is well respected by both patients and frontline staff .I breathed a sigh of relief when she was appointed as at least we had ,in her,a person aware of fundamental problems at the grassroot level.Able to look at the bigger picture and the ever pressing constraints of time and finances on an audit led ,target based ,disaffected hospital ...It seems that this mismanagement and creative fiddling of statistics goes back some time .Did another lamb need slaughtering ?...to be replaced by an idiot who knows nothing about health care and whose people skills are limited to caustic memo's telling us to work even harder or fear a workforce cutback .At least Dr Bene understood the bigger picture and I believe she took the job on because she felt she could do the job better and with more compassion than the preceeding group of faceless idiots .

An important question to be asked is why a former Chief Executive of this Hospital is now leading the Advancing Quality Agenda as Chief Executive at AqUA on behalf of the NHS - got the job there and is heading up the Mortality and quality initiatives in the North West and beyond- inspires great confidence doesnt it!! Questions need to go wider thatn Bolton and AqUA has some explaining to do too .

Calling in more help? Soon there will be more accountants, investigators, (no CID though) consultants, expert analysts, than patients. Make no mistake they will all be getting their heads together to cover their own backsides, look what happened at Stafford, after all the enquiries, they made the culprit head man of the whole NHS.

I totally understand your way of thinking but I believe the massaging of statistics was probably authorised and instigated at a level away from the clinicians .We have been poorly led at the hospital for years by inept top heavy ,overpaid managers .The terrible lack of consistency has resulted in a great lack of confidence and trust .Sad when it was this status ,so badly wanted that led to the avalance of poor decisions ,bad ideas and even poorer judgement .Instead of sacking or retiring a CEO at every crisis and at great cost an opportunity to redeem themselves should have been available (obviously heavily scrutinised) this may at least have calmed this rudderless ship .I think Dr Bene probably offered to leave temporarily for the sake of the hospital she cares about rather than become embroilled in more pointless and expensive finger pointing...

I just want to say to RBH you have let so many people down it is about time the whole place was investigated.I for one have been handed my death sentence by this hospital told time and time again nothing wrong.Appointments lost in the post surgery cancelled twice whilst im preped for theatre.No treatment due to records lost.I found out by accident in december 2012 i had infact got a disease that is killing me.I cannot even look my family in the face.I have tumours growning so bad my bones have broke.To the management sitting behind your desk pointing the finger of greed at everyone else but themselves.What words of wisdom have you got for me and my family i am just arragnging my funeral maybe you should come along in your lovely car im sure it will brighten your world.You are a disgrace blame the staff blame each other at the end of the day my life and many others were in the hands of RBH you were responsible for the spending to fund care the only thing you funded was your bank account.

Charlie your experience is truly shocking. My good thoughts go out to you and your loved ones. My wife nearly died at this shambles of a hospital a year ago - it was only my insistence that the diagnosis of a couple of doctors and a nurse consultant ( the latter on £50K a year who patronised us and refused to bring a consultant in to see my wife until I refused to leave her office until she did so ) was wrong and I wanted a specialist to see her. Within a minute of seeing my wife he agreed with my diagnosis and panic broke out. Another day and she would have been another mortality statistic. Not one word of apology from the medics involved.

This is the reality of the NHS under Nicholson; inhumane, dishonest & incapable of reform. CEO's who are truthfull are hounded out and gagged and those that remain doctor the figures.
It did not start under Cameron but if he fails to act he will pay the price.

People have mentioned Aqua- the whole point of Aqua is that members go round providing really rubbishy reports for each other, forgiving each other for mortality failings. Bene did the job in the Aqua report on Morecambe Bay mortality, and Aqua then produced an even more worthless report forgiving Blackpool's mortality- that was a year ago, but only last month was a proper (we hope) investigation into Blackpool mortality announced after the release of the Mid-Staffs. report.
The statement above is that the former Bolton CE became CE of Aqua, bit the Aqua website says nothing about who its executives are. All very dodgy.

All the political parties are achieving their desired goals, get the people scrapping for crumbs from the high table and lets tuck into the riches areselves. If Morris is as bad as painted then arraign him for corruption, sack him or shut up. Its time we got up off our knees and voted in a council for us.
