Specialist homeless and vulnerable adult and diabetes teams at Bolton NHS Foundation Trust have been shortlisted for two prestigious awards.

Led by Diabetes Specialist Nurse, Lynne Wooff, Bolton’s teams have been shortlisted for Service Improvement Project of the Year and the Innovation Award at the Diabetes Nursing Awards 2024.

Lynne said it is great to be recognised for all the work they do.

She said: “It’s an honour to have been shortlisted for two awards and for us to have been recognised for all the work we do to support those that are homeless to better manage their diabetes and to also carry out diabetes screening in this vulnerable cohort.

“The entire team work holistically with people, teams and groups throughout Bolton’s communities so that we can support those who need our help, and to improve their outcomes from diabetes related complications.”

Lynne and team have made incredible improvements for homeless people with diabetes in Bolton.

This has included introducing a new programme of health checks, they carry out screening sessions at Bolton’s three main hostels and Emmaus and have developed a specific screening support for homeless people attending the emergency department.

In the last 12 months, 89 per cent of diabetic service users had also received all nine diabetes care processes.

The teams have also supported 39 people, many of whom would have had limited diabetes management previously and be at high risk of serious complications.

The awards recognise outstanding commitment, innovation and excellence in diabetes care.

Diabetes Nursing Awards event organiser Emily Mayhew said: “We are thrilled to recognise remarkable individuals who have dedicated themselves to advancing diabetes care and education.

“Their innovative approaches and tireless efforts have made a significant difference in the lives of people living with diabetes.”

The Diabetes Nursing Awards will take place in London on Friday, June 7.
